- $75 for typical adult passport renewal
- $85 for children under 16
- $100 for age 16 or over first-time applicants including holders of a previous lost/stolen U.S. passports and holders of previous 5-year children’s passports
In order to obtain a replacement passport the following is needed:
- proof of U.S. citizenship (i.e. a previous passport, U.S. birth certificate, naturalization papers, etc.)
- two 2" x 2" (5 cm x 5 cm) photos on white background
3. DS-82 application form (use this form if: you can submit your most recent, undamaged passport; AND you were at least 16 years old when this passport was issued; AND this passport was issued in the last 15 years; AND you use the same name as on your most recent passport OR can submit documentation to reflect your name change)
or DS-11 application form (use this form if: you have never had a U.S. passport; OR you are under age 16; OR your most recent passport was issued more than 15 years ago; OR your most recent passport was lost or stolen)
4. Passport application fee paid to the U.S. Embassy cashier in U.S. dollars in cash (for fees see above)
5. Police report from local authorities (in case of lost or stolen passports)
6. DS-64 Form, Statement Regarding Lost or Stolen Passport (if applicable).
In addition to the documents mentioned above, in order to obtain a replacement passport for a minor under age 16, both parents or legal guardians must:
- present evidence of child’s relationship to Parents/Guardians: birth certificate with parents’ names, adoption decree with adopting parents’ names, etc.
- show valid parental identification (i.e., valid U.S. or foreign passport, drivers license, Alien Resident Card from BCIS, etc.)
- sign and take oath before the Consular Officer. If the second parent is not available to sign, the appearing parent must submit second parent’s notarized written statement of consent, Form 3053 - (including the child’s name and date of birth) authorizing passport issuance for the child or must present evidence of sole legal custody of the child.
Please note that the Department of State’s Bureau of Consular Affairs requires the personal appearance of all minors applying for U.S. passports in order to enhance the accurate identification of passport applicants and aid in the prevention of international child abduction and trafficking.
If your passport is valid but all visa pages are filled, the Consular Section can add new visa pages to your passport. You need to fill out Passport Amendment Application Form DS-4085 and present your U.S. passport. This service is free of charge.
